Payment is due at the time of your visit.
 

Insurance coverage varies based on your individual plan. We recommend verifying your mental health benefits, copays, deductibles, and any prior authorization requirements before your appointment.

You may be responsible for any costs not covered by your insurance plan. 

We understand that schedules can change. If you need to cancel or reschedule, please provide at least 48 hours’ notice.

Missed appointments or late cancellations may be subject to a fee of $100

You have the right to receive a “Good Faith Estimate” explaining the expected cost of your medical care.

Under federal law, healthcare providers are required to provide this estimate to patients who do not have insurance or who are not using insurance.

You may request a Good Faith Estimate before scheduling your appointment.
